Depends on if you are shipping internationally or domestic. In any case, I've only ever asked for shipping costs separate from the cost of the doll. After it's packed and weighed, then I give the shipping cost for which they have to agree before hand. I only charge the actual shipping costs...no handling fees. If you are in the US and using USPS, you might find a flat rate box that would work and would help save on costs.
Just because you CAN doesn't mean you SHOULD.
