However in the very last episode after the credits it implies she gets to choose which reality she wants to return to. However, I don't see anything that hints on which one she ends up choosing, other then the moon within a moon card was in the center (which would have to be Ukyo or nobody depending, as the other 4 are obviously color coded to their respective guys).
From my observations of human behavior the most recent boyfriend is the one she'd end up with, which would probably end up as the Stockholm Syndrome relationship with Toma, as they very clearly said the Ukyo relationship was impossible. Ukyo would've been my first choice if it hadn't been explicitly disallowed. She was willing to stay and die in the fire with him after all.

As for the heroine's true love---if you noticed, she was truly happy with Toma, and even Ukyo knew that---which is why he was so angry about her being with Toma. Additionally, when she was being carried up to the roof by Ukyo, her subconscious mind longed for the arms to be those of Toma---whom she was comfortable and at ease with always.
Ukyo is the catalyst of her world jumping, but as you said, the whole premise is that she CANNOT be with him---it's against fate and destiny---so that's impossible. Either she or he dies whenever they try to go together, which drove him to insanity and then caused him to try to kill her so that he wouldn't be the one to die. (His split crazy personality! :laughs: )
Also, in the game, Ukyo mentioned that she has many many worlds with Shin, a bunch with Ikki, a bunch with Kent, several with Wakka, none with himself, and ONLY ONE with Toma.
So, if you also take that to imply that the 'only one' route is the ONE TRUE world, again, it's Toma.
Additionally, he just wanted to protect her at all costs and in the game, she even crawled back into the cage after she escaped it, because when she went outside, Ukyo tried to kill her, Ikki's fangirls were after her, etc. She felt safer in the cage. ^^
The anime left out a lot of the details but still gave across the main idea.
Also, if you consider that since there are many worlds---all with the heroine in love with one of the 5 guys (Ukyo doesn't count, since they're not meant to be)---ALL the 5 guys can be counted as 'canon' for their particular 'heroine.'

From the game evidence and even the anime, it would appear to be Toma. He was the only one she really felt comfortable with and even, loved.
She flat out told Kent that 'the me of your world must care for you very much.' which directly says 'It's not me.' So he's out.
She's frightened of Shin in the anime---always cringing whenever he tried to kiss her. She was NOT happy with him, nor at ease. Ikki grew on her, but again, she wasn't 'sure' and she wasn't happy, plus he goes with Rika in all the worlds where he's not with the heroine. So that pretty much rules him out. Ukyo is automatically out by virtue of the whole game/anime's premise.
That leaves Toma, who is utterly dedicated to her, AND she seemed to reciprocate, wanting to find out if she loves him (having amnesia), being happy in the world with him (causing Ukyo to become even more angered) and actually calling out Toma's name, while snuggling against a chest (Unfortunately for her, it was Ukyo, who became very angry at her endearment, making him toss her off the roof with a vengeance! :laughs: ).
But again, the anime specifies a particular 'heroine' trying to find her place (it appears it should be Toma), but in the game, all the worlds are equally true---so pairing her with any of the guys is 'canon.' My sister loves Toma best, so we're happy that it really seems best and most accurate with him anyway. But any of them would be okay. (However, not really Ikki, because of Rika! It seems farfetched to just leave her behind, wishing 'congratulations' to the heroine for stealing her Ikki. Ha ha ha.)
So, after watching it, did you enjoy the anime? I thought it was pretty good---maybe a bit too 'fast paced' and skipping important elements, but mostly all good!!!
And probably on the dvd release, they'll put out 'multi-endings' where you'll see her with each guy. ^^
