I did this shot for an ADAD picture and it started me thinking about interviewing minions. But, what questions would need to be on the application?

Maybe something along these lines? ^.^

1. Your name
2. Your REAL name
3. How would you address your Master (ex: Your Evilness, Master/Mistress, Dragonlady/lord, My Love)
4. Do you or have you ever had a hunchback? (We do not disqualify for handicaps)
5. Can you follow simple instructions?
6. Will you be available for our service twenty-four hours a day, seven days a week?
7. Can you cook a decent meal without using poisons?
8. Do you require an excuse for school?
9. Do you now have or have ever had aspirations in being a Master in your own right?
10. What is your favorite Star Wars movie?
11. Do you now or have you ever believed that vampires sparkle when coming in direct contact with sunlight?
12. What would you do if a skateboarding ninja gerbil attacked you?
13. What would you do if your Master was trapped in a guillotine and had no way of escaping?
14. Are all your vaccinations current?
15. If references are asked for, could you provide them without digging up an unmarked grave?
